    “Transition” means to shift or change from one thing to another. There are many types of transitions throughout life. At Michigan Alliance for Families, “transition” refers to youth or young adults with disabilities moving from school into adult life. Did you know that transition planning is actually required by law? State and federal rules and regulations provide a framework with requirements and services along the way. This website is designed to help youth and young adults with disabilities, families and professionals understand their rights and responsibilities at each step of this important transition.
